Forms & Publications - New certification wording on Form W-4

Employers using electronic systems for employee submissions of Forms W-4 Employee's Withholding Allowance Certificate, substitute return developers, and software programmers must update their products to reflect the new employee signature certification ("jurat") that appears on the 2005 Form W-4.

The revised jurat reads as follows:

"Under penalties of perjury, I declare that I have examined this certificate and to the best of my knowledge and belief, it is true, correct, and complete."

No variances from the specified text are allowed.
